The UHMG model village concept aims at empowering communities to take charge of their health and take action for prevention and treatment (referral mechanism to health facilities) of common diseases and health situations that affect the family with special focus on mother and child. UHMG creates the network of health services through linking community services to health facility based services. At the center of this model is the Good Life! Clinic that acts as a referral point for the community based service.
